(Lebanon, PA) – Do you have 10 minutes? We are listening!
The Community Health Council of Lebanon County, in partnership with the REACH Project, Lebanon Valley Chamber of Commerce, and the United Way is asking you to participate in a community survey about your health and overall wellness.
The anonymous survey includes questions about health services, housing, transportation, and personal wellbeing. With your help, we can better understand the key problems and assets of our community. The survey takes about 10 minutes and gives you the opportunity to weigh in on important health topics.
A community health needs assessment (sometimes called a CHNA) is a popular tool used to assess a community’s current health status, needs, and issues. This information can help us develop a community health improvement plan by justifying how and where resources should be allocated to best meet our community needs.

- Increased understanding of public health and the connection between needs, resources, and activities
- Strengthened partnerships within state and local public health systems
Working alongside our community partners, we can and will use survey data to evaluate and develop programs that meets the needs of our community.
